This fall, KHEIR Clinic and KYCC have come together to provide medication-assisted treatment to patients. KYCC Rehabilitation Service Manager Hiroko Makiyama said, “The medication-assisted treatment, which has been proven effective through a variety of verifications, is a good model for healing a person through the combination of prescription and counseling of auxiliary drugs. We will safely treat patients with disabilities and help them return to their daily lives.”
Medication-assisted treatment is a method of treating drug addiction by assisting/using drugs. Under medication-assisted treatment, counseling and various complementary methods are combined with medication. Patients are assisted through drug prescriptions approved by the Federal Food and Drug Administration (FDA) and provided counseling and cognitive behavioral therapy to help them escape drug addiction and dependence.
Medication-assisted treatment is not only applied to opioids, but also to other drug or alcohol addiction treatments. As drug and alcohol addiction patients have increased, interest in medication-assisted treatment has also increased, and medical institutions that provide actual drug-assisted treatment are also becoming more prominent.
Prescribing these drugs requires either obtaining a federal permit or completing a federally established course for each drug. Medication-assisted treatment is supplemented by counseling, cognitive and behavioral treatments to help recover from drug addiction and reduce the risk of addiction relapse.
“The drugs prescribed by MAT include buprenorphine, methadone, and naltrexone, which have the effect of minimizing withdrawal symptoms and alleviating side effects, such as nausea, from the process of discontinuing opioids and alcohol,” said Eric Schluderberg, KHEIR Clinic Specialist. “These drugs work to normalize brain hormones, block the rewarding effects obtained through opioids and alcohol, suppress the desire to use drugs, and normalize body functions.”
Opioid addiction and abuse is a serious health crisis in our community. According to the National Center for Health Statistics (NCHS) under the Federal Centers for Disease Control and Prevention (CDC), as of 2017, there have been nearly 70,000 deaths from drug overdoses in the United States, of which close to 48,000 were from opioid overdoses.
With opioid addiction and abuse becoming more and more serious, medication-assisted treatment (MAT), which is a medical alternative treatment that helps to escape the pain caused by drug or opioid use disorder (SUD, OUD), has been gaining more attention.
KHEIR Clinic will provide medication-assisted treatment to Medi-Cal patients, and KYCC provides free recovery services to Medi-Cal patients and MyHealth LA members. “약물 중독 약물로 치료한다” 오피오이드 오남용 심각 … 약물 보조 치료(MAT) 대안
약물처방과 상담 병행 … 이웃케어·KYCC 협력 서비스 제공
(10 월 14 일) 오피오이드(opioid, 마약성 진통제) 중독 및 오남용이 심각하다. 연방 질병통제예방센터(CDC) 산하 국립보건통계센터(NCHS)에 따르면 2017 년 기준 약물 과다복용으로 사망한 미국인은 7 만 명에 육박하고 이중 오피오이드 중독으로 인한 사망자는 4 만 8000 명 가까이 된다.
이처럼 오피오이드 중독 및 오남용이 점점 심각해지고 있는 상황에서 약물 또는 오피오이드 사용 장애(SUD, OUD)로 인한 고통에서 벗어나도록 돕는 의학적 대체 치료인 약물 보조 치료(Medication-Assisted Treatment, MAT)가 주목받고 있다.
약물 보조 치료란, 말 그대로 약물을 보조/활용해 약물 중독을 치료하는 방법이다. 약물 치료와 함께 상담과 다양한 보완적 방법을 병행한다. 오피오이드에 중독되거나 의존하는 환자를 치료할 때 연방 식약국(FDA)이 허가한 약물 처방을 통해 치료를 돕고 상담과 인지행동치료를 병행해 환자가 약물 중독 및 의존에서 벗어나도록 하는 것이다. 약물 보조 치료는 오피오이드 뿐만 아니라 다른 약물이나 알코올 중독 치료에 적용되기도 한다. 오피오이드등약물및알코올중독환자가증가하면서약물보조치료에대한관심이높아지고있고실제약물보조 치료를 제공하는 의료기관도 늘고 있다. 한인사회에서는 이웃케어클리닉 (KHEIR Clinic, 소장 애린 박)과 한인타운청소년회관 (KYCC, 관장 송정호)이 협력해 환자들에게 약물 보조 치료를 제공하고 있다. 이웃케어클리닉의 에릭 슐루더버그 전문의는 “MAT 에서 처방하는 약물에는 오피오이드 및 알코올 중단 과정에서 오는 금단 증상을 최소화하고 메슥거림 등 부작용을 완화하는 효과가 있는 부프레노르핀, 메타돈, 날트렉손 등이 있다”며 “이들 약물은 뇌 호르몬을 정상화하고 오피오이드나 술을 통해 얻게 되는 행복 효과를 차단하며 약물 사용에 대한 욕구를 억제하고 신체 기능을 정상화하는 작용을 한다”고 설명했다.
이들 약물을 처방하려면 각각의 약물마다 연방정부 허가를 받거나 연방정부가 정한 과정을 이수해야 한다. 여기에 더해 상담을 통해 인지 및 행동 치료를 병행,약물 보조치료를 보완해약물중독에서 회복하고 약물 의존성에서 벗어나도록 도와 중독 재발 위험을 줄여주도록 하고 있다.
KYCC 재활서비스 히로코 마키야마 매니저는 “다양한 검증을 통해 효과가 입증된 약물 보조 치료는 보조 약물 처방과 상담을 병행해 한 사람을 치유하는 좋은 모델”이라며 “이웃케어 MAT 팀과 더욱 긴밀하게 협업해 약물 사용 장애가 있는 환자를 안전하게 치료하고 이들이 일상으로 복귀할 수 있도록 돕겠다”고 강조했다.
